You can find the Guide Voodoo Doll by defeating Voodoo Demons in the Underworld. Once you have received the doll, go over to the Lava in the Underworld and throw it in. This kills the doll, and give you the Wall of Flesh.
This boss is one of the strongest in Terraria, so be aware! He will occupy the entire screen and deal large amounts of damage.
